outputting lines from a file...

I want to ask user to either look up address and phone number, and then display the address or phone number, depending on which one they choose. I cant really figure out how to output the address or phone number. My file is as follows( this is how its supposed to look), and the code. Thanks for any suggestions or operators i could use!
